Step 6: Submit Lifeline and Link Up Worksheet (Form 497)
In order to receive Low Income support, an eligible telecommunications carrier (ETC) must provide information to USAC demonstrating that the ETC is complying with the rules set out in 47 C.F.R. §54.400 et seq.
ETCs file the Lifeline and Link Up Worksheet (Form 497) with USAC to be reimbursed for providing Lifeline, Link Up and Toll Limitation Support (TLS) to eligible households.
ETCs must file one Form 497 for each month for which they are claiming support. ETCs may either file quarterly or monthly.
